We would love to have your child here at our center, if you have decided that we are the right place for your family you may fill out the registration packet below and bring it with you. We can always provide you with a registration packet here at the center, in person, if you prefer that as well.
Each child will need their own enrollment forms completed. This for is for either of our locations.
Please complete this form only if your child has an allergy to any type of food that could possibly be served at our center. This form must be signed by your child’s doctor.
This form must be completed each year in January for each child. This allows us to update your child’s folder with current information about the child, medical conditions, parents, and emergency contacts.
We are on the USDA Food Program which requires us to have a completed form for each family on file.
This form must be completed by parents of our infant children at least once per month or anytime there is a change in the infants feeding schedule.
Burleson Child Development Center offers automatic payment processing by Credit Card FREE OF CHARGE! If you would like to pay via Credit Card or EFT or need to update the form of payment we have on file compelte this form.
This form is required to be on file with the center and to be kept up to date. You must have this form signed by your child’s physician and return it to us within 5 days of enrollment.
This form must be completed by parents of our infant children under the age of 12 months.
Our Parent Handbook includes information about our policies and procedures. There is also information about holiday parties, birthday parties, visitors at the center, and other things of that nature that many parents find useful.
